About The Position

Orchestrate continuum of care plans from admission through discharge by aligning clinical pathways with patient and family values while removing barriers to care. Evaluate medical necessity and level of care through continuous patient assessments and record reviews to ensure appropriate utilization of healthcare resources. Guide seamless post-acute care transitions by serving as the primary liaison between patients, families, and post-acute providers to secure optimal post-discharge services. Optimize financial and clinical resources through strategic collaboration with payers, advocating for maximum benefit utilization and efficient, high-value care delivery.

Requirements

Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Prior case management experience.

Nice To Haves

Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
