About The Position

The Registered Nurse Care Coordinator (RNCC) in UVA Health’s Digestive Health Clinic serves as a clinical connection point between patients, providers, and essential services. The role supports people with Crohn’s disease, ulcerative colitis, celiac disease, chronic abdominal pain, and other complex gastrointestinal conditions as they navigate diagnostic evaluation, treatment, and long-term disease management. The RNCC assesses changing patient needs, investigates clinical concerns, and translates provider care plans into coordinated action. The position combines patient relationships, independent nursing practice, specialty development, and a predictable hybrid work structure. The schedule is Monday through Friday during standard business hours, with no nights, weekends, or major holidays. A consistent hybrid model combines onsite clinical collaboration with remote work aligned to provider schedules. Schedules are generally established in advance, with flexibility expected to support clinic coverage when team needs change. The nurse works collaboratively with GI providers, specialty services, physicians, nurses, and clinic staff in a growing multidisciplinary specialty practice. The role offers opportunities to develop gastroenterology expertise, take ownership of complex patient needs, support evidence-based care, and pursue career development through education, mentorship, recognition, shared governance, and specialty certification resources. Relocation assistance may be available for qualified candidates.
