Requirements

Bachelors of Science in Nursing (BSN) from an accredited college of nursing required, or obtained within 5 years from date of hire for nurses with an Associate Degree in Nursing (ADN).
Successful completion of annual mandatory education requirements.
Minimum of two years of relevant clinical experience.
Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
Ability to multi-task and prioritize work assignments.
Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
Ability to think critically and problem solve.
Excellent negotiation skills, with the ability to address complex issues with physicians and staff across multiple departments.

Nice To Haves

Certification(s) in area of specialty preferred.
From National Certification Corporation (NCC) within 1 year of eligibility.
Case Management certification preferred.

Responsibilities

Provides appropriate care specific to the age of the customer/patient, ensuring understanding and comfort level of treatment.
Performs initial and ongoing assessments to determine patient needs, including medical, psychosocial, financial, and discharge needs. Screens admissions for complex case management and monitors caseload for variances.
Develops, reviews, revises, and individualizes the plan of care, collaborating with providers and clinical disciplines.
Implements interventions consistent with the plan of care, following nursing protocols and standards of care.
Performs technical skills competently and independently, demonstrating correct and safe technique in equipment use.
Maintains professional behavior, attitude, and interactions to achieve the organization's mission. Communicates effectively and maintains a professional appearance and work environment.
Initiates action to correct, prevent, or minimize risks to patients, families, or staff. Completes appropriate risk reports and communicates unresolved issues.
Acts professionally responsible for human and other resources, charges accurately for supplies and equipment, and partners with post-acute care providers.
Coordinates care by collaborating with physicians, interdisciplinary teams, and patients/families to develop comprehensive discharge plans. Establishes effective relationships and facilitates timely referrals.
Participates in Performance Improvement activities and assists with the update/development of critical paths and standing order sets. Participates in hospital-wide committees and initiatives.
