Requirements

  • Bachelors of Science in Nursing (BSN) from an accredited college of nursing required, or obtained within 5 years from date of hire for nurses with an Associate Degree in Nursing (ADN).
  • Successful completion of annual mandatory education requirements.
  • Minimum of two years of relevant clinical experience.
  • Current valid Registered Nurse (RN) License, valid compact multi-state license, or a temporary permit while awaiting licensure required for work state.
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
  • Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
  • Ability to multi-task and prioritize work assignments.
  • Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
  • Ability to think critically and problem solve.
  • Excellent negotiation skills, with the ability to address complex issues with physicians and staff across multiple departments.

Nice To Haves

  • Certification(s) in area of specialty preferred.
  • From National Certification Corporation (NCC) within 1 year of eligibility.
  • Case Management certification preferred.

Responsibilities

  • Provides appropriate care specific to the age of the customer/patient, ensuring understanding and comfort level of treatment.
  • Performs initial and ongoing assessments to determine patient needs, including medical, psychosocial, financial, and discharge needs. Screens admissions for complex case management and monitors caseload for variances.
  • Develops, reviews, revises, and individualizes the plan of care, collaborating with providers and clinical disciplines.
  • Implements interventions consistent with the plan of care, following nursing protocols and standards of care.
  • Performs technical skills competently and independently, demonstrating correct and safe technique in equipment use.
  • Maintains professional behavior, attitude, and interactions to achieve the organization's mission. Communicates effectively and maintains a professional appearance and work environment.
  • Initiates action to correct, prevent, or minimize risks to patients, families, or staff. Completes appropriate risk reports and communicates unresolved issues.
  • Acts professionally responsible for human and other resources, charges accurately for supplies and equipment, and partners with post-acute care providers.
  • Coordinates care by collaborating with physicians, interdisciplinary teams, and patients/families to develop comprehensive discharge plans. Establishes effective relationships and facilitates timely referrals.
  • Participates in Performance Improvement activities and assists with the update/development of critical paths and standing order sets. Participates in hospital-wide committees and initiatives.
