About The Position

Ascension St. Vincent is seeking a Registered Nurse for Bed Placement in the Care Coordination Care Center department. This is a full-time night position. The role involves managing the bed board, assigning beds to overnight patients, and coordinating with all units and their leadership. It is a non-bedside role focused on implementing and monitoring patient care plans, assessing and coordinating patient discharge planning needs, and noting/carrying out physician and nursing orders.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Responsibilities

Work the bed board and assign the beds to all patients who stay overnight.
Implement and monitor patient care plans.
Monitor, record, and communicate patient condition as appropriate.
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
Note and carry out physician and nursing orders.
Assess and coordinate patient's discharge planning needs with members of the healthcare team.
