About The Position

The Registered Nurse - Admission Line Coord facilitates and coordinates acute transfers into Trinity Health Grand Rapids (THGR) in accordance with clinical standards and guidelines. The role manages calls from acute care facilities, physician offices, and long-term care facilities seeking admission to THGR or evaluation in the Emergency or OB department. The coordinator serves as a resource for sending facilities and THGR attending or admitting physicians, helps identify the appropriate admitting service, facilitates clinical communications and transfer coordination, and collaborates on patient placement.

This is a part-time position of 24 hours per week on the night shift, 7:00 PM–7:30 AM.

Requirements

• Bachelor’s degree in nursing or another health-related field.
• Current Michigan Registered Nurse license.
• Active BLS certification.
• Current knowledge of patient care practices and health care systems.
• Five years of current and progressive clinical and leadership experience in an acute care setting, including direct patient care and staff education.

Responsibilities

• Answer and respond to incoming calls regarding patient transfers for admission or evaluation at Trinity Health Grand Rapids.
• Serve as a resource for sending facilities and THGR attending and admitting physicians.
• Provide direction and assistance in identifying the appropriate admitting service for a patient transfer or evaluation.
• Facilitate ED-to-ED communication and physician-to-physician conversations for transfers and admissions.
• Collaborate with the Clinical Resource Coordinator to plan patient placement.
• Facilitate receipt of admitting orders from the admitting physician when needed.
