About The Position

The Registered Nurse (RN) - Assertive Community Treatment Team (ACTT) plays a critical role in delivering comprehensive, community-based mental health care to individuals with severe and persistent mental illnesses. This position focuses on providing holistic nursing assessments, medication management, and therapeutic interventions within clients' homes and community settings to promote recovery and improve quality of life. The RN collaborates closely with multidisciplinary team members to develop and implement individualized care plans that address both physical and mental health needs. By engaging clients in their own care and supporting their social integration, the RN helps reduce hospitalizations and enhances community functioning. Ultimately, this role is essential in bridging clinical expertise with compassionate outreach to support vulnerable populations in achieving stability and independence.

Requirements

  • Current registration as a Registered Nurse with the appropriate provincial regulatory body in Canada.
  • Bachelor of Science in Nursing (BScN) or equivalent nursing education.
  • Minimum of two years of nursing experience, preferably in mental health or community health settings.
  • Valid CPR certification and up-to-date immunizations as per employer requirements.
  • Ability to work independently and as part of a multidisciplinary team in community environments.
  • Strong clinical assessment and critical thinking skills.
  • Communication skills for building therapeutic relationships with clients, families, and team members.
  • Organizational skills for managing multiple caseloads, coordinating care plans, and maintaining accurate documentation.
  • Problem-solving abilities to adapt interventions in dynamic community settings and respond to crises promptly.
  • Cultural competence and empathy to respect client diversity and promote inclusive, client-centered care.

Nice To Haves

  • Experience working within Assertive Community Treatment Teams or similar community mental health programs.
  • Additional certifications in mental health nursing or psychiatric nursing.
  • Training in crisis intervention, trauma-informed care, or motivational interviewing.
  • Familiarity with electronic health records and documentation systems used in community health.
  • Knowledge of local community resources and social services relevant to mental health populations.
  • Access to a reliable vehicle is necessary.

Responsibilities

  • Conduct comprehensive nursing assessments and monitor clients’ physical and mental health status regularly.
  • Administer and manage medications, ensuring adherence and monitoring for side effects or complications.
  • Develop, implement, and evaluate individualized care plans in collaboration with the ACTT multidisciplinary team.
  • Provide crisis intervention and support to clients in community and home settings as needed.
  • Educate clients and their families about mental health conditions, treatment options, and wellness strategies.
  • Coordinate with external healthcare providers and community resources to ensure continuity of care.
  • Document all nursing activities, client progress, and clinical observations accurately and timely.
  • Participate in team meetings, case conferences, and ongoing professional development activities.
