About The Position

The Registered Nurse (RN) within the Assertive Community Treatment Team (ACTT) plays a critical role in delivering comprehensive, community-based mental health care to individuals with severe and persistent mental illnesses. This position focuses on providing holistic nursing assessments, developing individualized care plans, and administering treatments that promote recovery and improve quality of life. The RN collaborates closely with multidisciplinary team members to ensure coordinated care, crisis intervention, and ongoing support tailored to each client’s unique needs. By engaging clients in their own environments, the RN helps to reduce hospitalizations and supports community integration. Ultimately, this role aims to empower clients through compassionate, evidence-based nursing care that fosters independence and wellness in the community setting.

Requirements

  • Current registration in good standing as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO).
  • Bachelor of Science in Nursing (BScN) or equivalent nursing education.
  • Experience in mental health nursing or community health nursing.
  • Knowledge of crisis intervention techniques.
  • Ability to work independently and as part of a multidisciplinary team in community settings.
  • Valid driver's license and access to a reliable vehicle are required for regular travel to client locations within the community.

Nice To Haves

  • Experience working within Assertive Community Treatment Teams or similar community mental health programs.
  • Additional certifications in psychiatric nursing, mental health, or substance use disorders.
  • Familiarity with electronic health records and documentation systems.
  • Training in trauma-informed care and recovery-oriented practice.
  • Bilingualism in English and French or other languages relevant to the community served.

Responsibilities

  • Conduct thorough nursing assessments and monitor clients’ physical and mental health status regularly.
  • Develop, implement, and evaluate individualized nursing care plans in collaboration with the ACTT multidisciplinary team.
  • Administer medications and treatments safely while educating clients and their families about health management.
  • Provide crisis intervention and support to clients experiencing acute episodes or challenges in the community.
  • Coordinate with community resources and healthcare providers to facilitate comprehensive care and follow-up.
  • Document all nursing activities, client progress, and care plans accurately and in a timely manner.
  • Participate in team meetings, case conferences, and continuous professional development activities.
