Accountable to the Clinical Program Director, Schizophrenia and Community Integration Service, the Clinical Manager for ACT Teams is responsible for the overall operations of the assigned areas and management of the human and fiscal resources required to meet the patient care needs of those clinical areas, as well, as for the integration of quality, staff safety and patient safety initiatives. The Clinical Manager takes an active role in strategic planning, program development, volume management and quality monitoring. The Manager promotes the integration and ongoing advancement of standards and best practices, educational activities and promotion of research activities. As a leader, the Manager provides, facilitates and promotes the best possible professional service to the patients and employees of these clinical areas, as well as to the organization and community. The Manager assumes responsibility for the designated areas as assigned and is accountable for the hiring and effective management of all direct report staff and for financially managing the cost centres for assigned units. The Manager ensures that the team works collaboratively with other community based mental health and addiction services, regional/local hospitals, social services, Public Guardian & Trustees and other relevant community resources based on patient need. The manager is responsible for Haldimand Norfolk ACTT and Brant ACTT that offer expert, comprehensive and intensive care provided on an outreach basis and based on provincial care standards for ACT. The teams serve a population group of adults with heterogeneous diagnoses of serious and persistent mental illness, predominantly psychotic disorders, and significant substance use disorders with the additional complexity related to psychosocial factors including housing, finances, supports, and serious medical co-morbidities and legal complexities. Many patients have active Community Treatment Orders. All ACT teams are based in the community. The Clinical Managers are responsible for operations headquartered at community offices and that serve on an outreach basis wide geographies. Haldimand Norfolk ACTT services Delhi, Langton, Long Point, Port Rowan, Port Dover, Caledonia, the New Credit Reserve, Jarvis, Hagersville, Simcoe, Dunnville and Low Banks. Brant ACTT serves Brantford, Brant County (Burford, Oakland, Paris and Six Nations Reserve).
