About The Position

Accountable to the Clinical Program Director, Schizophrenia and Community Integration Service, the Clinical Manager for ACT Teams is responsible for the overall operations of the assigned areas and management of the human and fiscal resources required to meet the patient care needs of those clinical areas, as well, as for the integration of quality, staff safety and patient safety initiatives. The Clinical Manager takes an active role in strategic planning, program development, volume management and quality monitoring. The Manager promotes the integration and ongoing advancement of standards and best practices, educational activities and promotion of research activities. As a leader, the Manager provides, facilitates and promotes the best possible professional service to the patients and employees of these clinical areas, as well as to the organization and community. The Manager assumes responsibility for the designated areas as assigned and is accountable for the hiring and effective management of all direct report staff and for financially managing the cost centres for assigned units. The Manager ensures that the team works collaboratively with other community based mental health and addiction services, regional/local hospitals, social services, Public Guardian & Trustees and other relevant community resources based on patient need. The manager is responsible for Haldimand Norfolk ACTT and Brant ACTT that offer expert, comprehensive and intensive care provided on an outreach basis and based on provincial care standards for ACT. The teams serve a population group of adults with heterogeneous diagnoses of serious and persistent mental illness, predominantly psychotic disorders, and significant substance use disorders with the additional complexity related to psychosocial factors including housing, finances, supports, and serious medical co-morbidities and legal complexities. Many patients have active Community Treatment Orders. All ACT teams are based in the community. The Clinical Managers are responsible for operations headquartered at community offices and that serve on an outreach basis wide geographies. Haldimand Norfolk ACTT services Delhi, Langton, Long Point, Port Rowan, Port Dover, Caledonia, the New Credit Reserve, Jarvis, Hagersville, Simcoe, Dunnville and Low Banks. Brant ACTT serves Brantford, Brant County (Burford, Oakland, Paris and Six Nations Reserve).

Requirements

Masters' degree preferred in a related Healthcare discipline - e.g. nursing, social work.

Responsibilities

Hires, promotes, disciplines and terminates staff, as required.
Manages employee relations issues, promoting a teamwork environment where staff can interact productively, inter-professionally, and efficiently.
Develops, implements and communicates performance measures; conducts regular performance appraisals of direct reports, providing ongoing coaching and feedback.
Ensures adherence to provincial ACT model of service delivery.
Fosters an environment which facilitates the use of evidence informed practices, ongoing professional development, and interdisciplinary care, clinical documentation, research and program evaluation.
Liaises with the teams’ psychiatrists to ensure quality treatment and care of patients and their families.
Ensures excellence of care within clinical areas and facilitates the implementation of key performance indicators by optimizing data sources including through Dovetale
Develops and implements appropriate orientation programs for new staff. Provides coaching, supervision and mentoring
Evaluates staff training needs on an ongoing basis. Sources and recommends appropriate external and/or develops internal programs to address these needs.
Fosters effective employee and labour relations through knowledgeable interpretation and implementation of hospital policies and collective agreements (ONA, OPSEU, CUPE). Consults with HR as needed/required.
Sets and manages staff work schedules; monitors attendance of all direct report staff and ensures expectations of attendance management program are adhered to.
Liaises with OH&S/HR to develop return to work plans for staff who require accommodation(s).
Completes and authorizes appropriate Payroll and Human Resources documentation within expected timelines.
Ensures ongoing assessment and reduction of risk for staff working both in the office and out in the community with regular debriefing with staff regarding interactions, intake and crisis response.

Oversees referral intake and registration for outpatients
Manages the development, implementation, evaluation and maintenance of quality improvement initiatives which are in alignment with established SJHH standards.
Directs department practices and processes to ensure excellence in client care delivery in accordance with MOHLTC and provincial Assertive Community Treatment Technical Advisory Panel policy framework and standards.
Commits to, develops and evaluates an optimal learner environment reflective of the program's position within an academic health science centre.
Engages in a review of all complaints/compliments for assigned area including investigation and resolution.
Provides expertise via consultation on the collection, analysis and dissemination of patient and family satisfaction data.
Manages financial resources allocated to portfolio within budget parameters; monitors targeted volumes and other relevant data.
Contributes to corporate efforts and initiatives, as appropriate, to enhance SJHH’s mission, values and goals.
Represents the unit/department internally on SJHH committees, as required, and SJHH in the external community at conferences, workshops, etc.
Participates in quality reviews including QCIPPA reviews as required.
Reviews and ensures completion of all online incident reports for the program.
Performs cross-functional and/or other duties consistent with the job classification, as assigned or requested.
Provides managerial coverage of other areas as needed for vacation/sick-time relief.
Provides manager-on-call duties.
Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contraventions of the Act, in a timely manner.
Ensures the collection and reporting of financial and service data, including the monthly, quarterly and year-end reporting of waitlist, MIS data and required information for through Ontario Health Centre of Excellence.
Develops and maintains partnerships with key community stakeholders including local hospitals, mental health and addiction agencies, social services, housing partners, ODSP, PGT etc. and represents team/program at various relevant community-based committees.
