About The Position

Montefiore Bronx Health Collective (BHC) is a Federally Qualified Health Center located in the South Bronx offering innovative, comprehensive primary care to communities challenged by social inequities and health disparities. We provide care across the lifespan, including primary care for children and adults; prenatal and reproductive health services; specialty referral management; integrated behavioral health; mental health counseling; case management and entitlements assistance; and health education. We seek a mission-driven Registered Dietitian to join our interdisciplinary team and support patients with culturally responsive, evidence-based nutrition care across the life course. The Registered Dietitian provides medical nutrition therapy and health education to patients with a focus on chronic disease management, prenatal and postpartum care, infant feeding, and preventive health. The role includes both individual counseling and group-based care, with opportunities to support innovative models such as group prenatal care and early parenting programs.

Requirements

  • Registered Dietitian Nutritionist (RDN) credential required
  • New York State certification (CDN) required or eligible
  • Experience in community health, primary care, or Federally Qualified Health Centers preferred
  • Experience with diabetes care, prenatal/postpartum nutrition, and/or lactation support strongly preferred
  • Comfort working with diverse, multilingual populations and supporting patients affected by social drivers of health

  • Bilingual (Spanish/English) preferred

Responsibilities

  • Provide individualized nutrition assessments, counseling, and follow-up for patients across the lifespan
  • Deliver diabetes nutrition education and support patient self-management in collaboration with primary care and behavioral health teams
  • Support prenatal and postpartum patients with nutrition counseling, including gestational diabetes, healthy weight gain, and micronutrient needs
  • Provide lactation and infant feeding support, including breastfeeding education and counseling
  • Contribute to quality improvement initiatives and population health efforts related to nutrition and chronic disease outcomes
  • Oversee BHC’s internal food bank
  • Coordinate with community nutrition partners, including the Department of Health and local food banks, and share opportunities for community program participation with BHC leadership.
