About The Position

Excel at Woodbury, located in Nassau County, is hiring a full-time Registered Dietitian. The role involves assessing patients’ nutritional status and needs, developing and evaluating nutrition care plans, providing nutrition counseling, and working with patients, families, and the interdisciplinary health care team. Duties also include weight monitoring, MDS documentation, discharge planning, and menu planning as needed. The position is in person and scheduled Monday through Friday, 9:00 AM to 5:00 PM. The listed annual salary is $85,000–$90,000.

Responsibilities

Evaluate patients’ nutritional status by reviewing information from electronic medical records, hospital discharge papers, the interdisciplinary health care team, and family members.
Apply nutrition screening and assessment knowledge to specific patient populations to define their nutritional needs.
Document pertinent findings in the medical record according to policy.
Provide nutrition counseling to patients and family members as indicated.
Implement and evaluate nutrition care plans and recommend changes as needed.
Adapt and apply guidelines, protocols, and professional standards of care to patients’ changing needs using professional judgment.
Evaluate the need for enteral and parenteral nutrition support and provide recommendations as indicated.
Participate in interdisciplinary care plan meetings and communicate effectively with health team members.
Complete Minimum Data Set (MDS) documentation as requested by the facility.
Assist with discharge planning.
Plan menus as needed.
Perform assessments, care planning, and weight monitoring.
