Provides nutrition education and care plans in the pediatric office setting. Plans, directs, and provides nutrition education and counseling to Parkside Pediatrics patients (infancy to 21 years) and their caregivers in an outpatient, office setting. Conducts nutrition assessments, creates interventions, and monitors growth, nutritional status, and treatment outcomes through ongoing follow-up appointments. Educate patients and caregivers on age-appropriate nutrition recommendations and feeding strategies. Provide nutrition education and coordination of care for various pediatric nutrition needs. Create and distribute educational materials tailored to pediatric populations and caregiver needs. Counsel families using motivational interviewing and family-centered approaches. Works both autonomously and collaboratively. Communicates with providers, external specialists, and the patient’s family to ensure continuity of care. Completes and maintains timely and accurate clinical documentation in accordance with regulatory and organizational standards. Takes ownership of ensuring the Parkside Way is experienced by Parkside patients, families, and internal team members. Promotes an authentic, transparent work environment with Parkside team members. Demonstrates excellent time management techniques. Attends meetings as scheduled. Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations.
