About The Position

Senior Care Options by Parker is seeking a Licensed Social Worker (LSW) for in-home geriatric counseling. This is a part-time position requiring a minimum of 16 client hours weekly. The role involves providing customized Geriatric Care Management services and Behavioral Health services to individuals in their homes and communities across Middlesex, Monmouth, and Ocean Counties. The goal is to support participants in maintaining independence in their own environment.

Requirements

• Master's degree required.
• Valid LSW in the state of NJ required.
• NJ driver's license in good standing required.
• Flu vaccine required during flu season.
• EHR knowledge preferred.

Nice To Haves

• Two (2) years of counseling experience preferred.

Responsibilities
