About The Position

The Regional Registered Nurse (RRN) plays a crucial role in coordinating health care services for Care Design NY (CDNY) Care Coordination Organization/Health Home (CCO/HH) members. This position involves a team-based approach to providing health services to individuals with intellectual/developmental disabilities (I/DD) through partnerships with individuals, their caregivers/families, community resources, and physicians. The RRN facilitates a person-centered approach to holistic service planning across settings to ensure coordinated, high-quality care. This role is a key part of the team supporting members and families with access to and ongoing support for all health care services. The RRN serves as an expert resource on health care supports and services, collaborating with team members for a holistic approach to care. This includes understanding CMS Medicare-Medicaid Plan, NYSDOH CCO, and OPWDD Reporting Requirements to assess CDNY performance and improve quality outcomes. Requirements

  • A Registered Nurse licensed to practice in New York state required
  • Excellent organizational, interpersonal, and verbal and written communication skills required; ability to attend to detail, organize tasks/priorities and complete with accuracy
  • Exemplary written and oral communication skills
  • Proficiency in navigating through electronic record software programs

Nice To Haves

  • BSN is preferred
  • 2+ years of experience in health services preferred
  • 2+ years of experience collaborating with people with IDD preferred
  • 3+ years of work experience in a managed care plan or health home is preferred
  • Experience working collaboratively with various departments in fast-paced environment

Responsibilities

  • Provide effective communication to improve health literacy.
  • Work with care managers to assist with the development of Life Plans based on mutual goals with the individual, individual's circle of support, and provider's plan as appropriate.
  • Function as a resource to support care managers who need assistance with complex or problematic health care issues with members.
  • Participate in complex care case reviews, Clinical Rounds, and other teams as needed to problem solve and assist those members most in need of a comprehensive and creative approach.
  • Assist in monitoring the outcomes from urgent care and emergency room visits, hospitalizations, discharge planning, and post-hospitalization plans to reduce further re-occurrences.
  • Support in the training of staff on health care issues, medications, and medication management and proactive practices.
  • Identify and collaborate when inadequate health care is provided to the member to come to a better level of care.
  • Ensure adherence to regulatory requirements such as CMS, NYS DOH Health Home, NYS OPWDD requirements.
