Jewish Family Services (JFS) is seeking a compassionate, organized, and mission-driven Refugee Health Screening Case Manager to support the health and wellness of newly arrived refugees through timely, culturally responsive medical screening coordination and case management services. The Refugee Health Screening Case Manager coordinates and facilitates domestic health screenings to ensure communicable diseases of public health importance and other health concerns are identified and addressed through timely screening, referral, follow-up, and care coordination. This position ensures screenings are initiated within 30 days of arrival and completed within required state timelines. As part of the JFS Health Team, this position works collaboratively across refugee health programs and is expected to support the broader Refugee Health Promotion Program, contribute to integrated service delivery, and assist in filling service gaps as needed to support program continuity and client needs. This position works closely with partner clinics and healthcare providers, the Office of Global Michigan, and other community partners to support timely reporting and documentation through the Michigan Disease Surveillance System (MDSS), while promoting equitable access to healthcare and successful integration for newly arrived refugee families.
