About The Position

Guelph Community Health Centre is seeking a motivated and organized Health Promotion Officer to support our health promotion, social prescribing and demographic data collection activities over the summer. This role is ideal for a self-starter with an interest in health promotion and community development and a passion for health equity. Working closely with the Health Promoter as part of an interdisciplinary team, the Health Promotion Officer will assist with program planning, facilitation, promotion, data collection and support for various programs.

Requirements

  • Be between 15 and 30 years of age at the start of employment
  • Be a Canadian citizen, permanent resident, or person with refugee protection status (international students are not eligible)
  • Have a valid Social Insurance Number and be legally entitled to work in Canada
  • Self-starter with a strong interest in health promotion, community development, social prescribing, health equity and community wellbeing.
  • Strong written and verbal communication skills
  • Organized, detail-oriented, and able to manage multiple tasks
  • Commitment to a high degree of confidentiality is essential
  • Comfortable working both independently and as part of a small team
  • Basic computer skills (Microsoft Office / Google Workspace)
  • Ability to work with equity-deserving groups in a trauma-informed, anti-oppressive, asset-based manner
  • Patient, reliable, and welcoming, with a genuine interest in the well-being of others

Nice To Haves

  • Work, volunteer or coursework in health promotion, community development, social prescribing, health equity and community wellbeing is considered an asset
  • Familiarity with PS Suite or other Electronic Medical Record system is considered an asset
  • G licence and access to a reliable vehicle is considered an asset

Responsibilities

  • Facilitating or co-facilitating (with volunteers) health promotion programs
  • Assisting with administration and coordination tasks related to health promotion projects
  • Attending and supporting community program meetings and activities as needed
  • Supporting health promotion quality improvement, evaluation and documentation tasks
  • Welcoming, signing in and making reminder phone calls to primary care clients
  • Collecting client socio-demographic data and updating electronic medical records
