About The Position

The Referral Tracking Coordinator is responsible for maintaining ongoing tracking and appropriate documentation on referral status. This role involves regularly contacting specialty offices and organizations to obtain consult reports and assisting patients with scheduling appointments. The position requires obtaining, reviewing, and requesting authorization and referrals as requested by the provider, monitoring and tracking patient referrals, and documenting all efforts in i2i systems and EMR. The coordinator will contact patients to ensure specialist appointments are scheduled, follow up to ensure appointments are kept, and obtain consult reports to close the referral loop. All communication regarding authorizations/appointments must be documented in a communication log within the patient's electronic health record. The role adheres to established criteria and time frames for processing and following up on referral requests and communicates with the requesting provider for any identified issues or amendments. The Referral Tracking Coordinator serves as a liaison between patients, clinic staff, providers, and outside agencies, and performs other duties as assigned.

Requirements

  • High School Diploma or GED.
  • Minimum of 3-5 years of experience in a medical office setting, or equivalent experience.
  • Bilingual in Armenian and English required.
  • Ability to find solutions when barriers are identified.
  • Strong documentation skills.
  • Ability to multi-task and prioritize when needed.
  • Ability to independently seek out resources and work collaboratively.
  • Ability to read, understand and follow oral and written instructions.
  • Experience and work ethics that supports working within a high functioning, team-oriented environment.
  • Demonstrates a willingness and ability to work under supervision.
  • Ability to develop and maintain good working relationships with staff.
  • Ability to use computer and learn new software programs.
  • Excellent interpersonal skills reflecting clarity and diplomacy and the ability to communicate accurately and effectively with all levels of staff and management.
  • Demonstrates ability to work in a regulatory climate that includes oversight of state and federal entities, payer contracts etc.
  • Possesses ability to communicate effectively, both verbally and in writing.
  • Possesses genuine respect for others and acceptance of their individual social and cultural traits.
  • Proficient knowledge of Microsoft Outlook.
  • Able to travel and attend professional meetings, conferences, trainings and clinic sites.
  • Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams.

Responsibilities

  • Performs other duties, as assigned.
