About The Position

As a Patient & Referral Services Representative at Ohio's Hospice, you will be an important first point of contact for patients, families, referral sources, and care teams across our service areas. This role is for someone who is compassionate, service-oriented, organized, and comfortable working in a fast-paced environment. It allows you to use your communication and customer-service skills to make a meaningful difference for patients and families during important moments in their lives.

Requirements

High school diploma or GED.
Strong computer and data-entry skills.
Excellent communication and customer-service skills.
Valid driver's license and a safe driving record.
Proof of required automobile insurance.
Ability to travel in varying weather conditions when necessary.
Ability to successfully complete a criminal background check and 10-panel drug screening.

Nice To Haves

At least two years of customer-service experience.

Responsibilities

Answer incoming calls and inquiries with professionalism, empathy, and exceptional customer service.
Gather, verify, and accurately document patient and referral information.
Explain hospice services clearly and answer questions from patients, families, healthcare providers, and community partners.
Maintain accurate, complete, and timely documentation of all interactions.
Work collaboratively with team members and provide support to other departments as needed.
Participate in quality-improvement initiatives designed to enhance the patient and family experience.
Assist with onboarding and training new team members when assigned.
