About The Position

CHCW is seeking a full-time Referral Coordinator to coordinate patient referrals and help patients access the care they need. The role supports healthcare providers and works with clinic teams, insurance companies, and specialists to ensure referrals are processed accurately, efficiently, and in a timely manner. Requirements

• High school diploma or GED.
• Two years of related experience.
• Valid driver’s license and auto insurance.
• Required immunizations before the start date: COVID, Hepatitis B (3), Tdap, MMR (2), Varicella (2), TB/PPD, and influenza.
• Pre-employment drug screen.

Nice To Haves

• Bilingual Spanish/English is preferred; candidates must pass ALTA to receive the bilingual differential.
