Requirements

Experience working with underserved populations or in social services case management or direct patient care.
Strong oral and written communication skills.
Experience using EPIC or other EHR systems.
Excellent interpersonal and multitasking abilities.
Self-motivated, accountable, and able to take constructive feedback.
Must be able to pass a background check, including fingerprints.

Responsibilities

Connecting patients to specialized services and ensuring they receive the care they need.
Collaborating closely with medical providers, behavioral health staff, and community partners to coordinate both internal and external referrals.
Scheduling appointments and facilitating follow-up care.
Managing incoming phone calls from patients and clinics.
Verifying insurance eligibility.
Guiding uninsured patients to community resources.
Overseeing various voucher programs, such as the IHC Diagnostic Service Voucher and HCI Colonoscopy Voucher.
Providing trauma-informed, patient-centered support.
Engaging with patients, staff, and community health centers.
Notating all interactions to enhance collaboration and improve patient services.
