About The Position

The Patient Referral Coordinator oversees and coordinates patient referrals to ensure timely, accurate, and seamless care and follow-up. This role works collaboratively with providers, patients, and external partners to facilitate the referral process, maintain precise documentation, and promote continuity of care across the patient journey.

Requirements

High school diploma or equivalent preferred
2-3 years of experience in a clinic setting preferred
detail-oriented, adaptable professional with strong communication skills
comfortable with EMR systems
works well with others
committed to accuracy, confidentiality, and a patient-centered experience

Responsibilities

Coordinate and track patient referrals from initiation through completion, ensuring timely and seamless care
Verify patient demographic, insurance, and clinical information, and obtain prior authorization as needed
Communicate with patients to explain referral details, next steps, and assist with scheduling appointments and follow ups
Maintain accurate documentation in the electronic medical record, including referral status and related communications
Collaborate with providers, clinical teams, and external partners to support effective care coordination and continuity of care
Performs a variety of tasks and other duties as assigned
