The Referral Coordinator I is part of Oak Street Health’s Patient Experience & Service Operations team and is responsible for managing day-to-day referrals and diagnostic orders so patients can access specialist care in a timely manner. The role works with center teams, payer partners, patients, and other internal and external partners to support a high-quality patient experience. This is a remote position. The employee must have proficient computer skills, reliable high-speed internet and hardwired equipment, a private and distraction-free work environment, and reliable dependent care during working hours. Employees located within 50 miles of an Oak Street Health location may need to provide their own transportation for on-site support. The role also requires participation in classroom-style remote training and the professionalism and reliability expected in a remote work environment. The progression from Referral Coordinator I to Referral Coordinator II requires at least six months in the role, strong problem-solving and communication, a thorough understanding of customer needs, a desire to learn, exceptional performance metrics for at least three consecutive months, and proven reliability and satisfactory attendance. Performance measures include call handling, unavailable time, quality, schedule adherence, and CareReq responsiveness. Service Delivery levels I through III form a structured progression toward broader knowledge, responsibility, and expertise. This is a full-time role with 40 anticipated weekly hours. The typical pay range is $17.00–$31.30 per hour; the actual offer depends on factors including experience, education, geography, and other relevant considerations. The role is eligible for a benefits package, subject to eligibility.
