About The Position

CancerCare Manitoba is a provincial agency that provides clinical and academic services and leadership for cancer control and the treatment of blood disorders. The Referral Assessment Nurse works within the centralized referral team, utilizing nursing knowledge and skills to triage, assess, and coordinate new patient referrals to CancerCare Manitoba. This role involves communication and collaboration with the Referral Office team, multidisciplinary teams, referring communities, and patients to ensure timely access to appropriate care. The nurse will also participate in wait time data collection processes.

Requirements

  • Current registration with College of Registered Nurses of Manitoba (CRNM)
  • Excellent interpersonal, communication, organizational and problem-solving skills
  • Ability to work both independently and as a member of a multi-disciplinary team
  • Strong understanding of patient-centred care
  • Basic computer skills including keyboarding, data entry, email and internet use
  • Physical and mental health to fulfill the responsibilities of the position
  • Satisfactory employment record
  • Must be legally able to work in Canada

Nice To Haves

  • Bachelor of Nursing
  • Three (3) years nursing experience within ambulatory/oncology
  • Knowledge of CancerCare Manitoba Referral process and triage experience
  • Proficient with MS Word/Excel/Outlook
  • Experience with electronic charting
  • Experience within the referral office or a referral office setting
  • Ability to speak French
  • Certified Oncology Nurse [CON(C)]
  • Commitment to ongoing professional development.

Responsibilities

  • Triage, assess, and coordinate new patient referrals to CancerCare Manitoba.
  • Communicate and collaborate with the Referral Office team, multidisciplinary team, referring community, and patients.
  • Ensure patients are seen in a timely manner by the appropriate Disease Site service provider.
  • Ensure patients undergo appropriate diagnostic tests.
  • Ensure patients have access to support as they enter CancerCare Manitoba.
  • Participate in wait time data collection processes.
