SUMMARY RS EDEN’s Recovery Services Treatment Coordinator assumes a shared responsibility for the development and implementation of treatment support and assigned goal plans for each client under the direction of the RS Support Services Director and Clinical Supervisor. The Recovery Services Treatment Coordinator provides ongoing assessments, monitoring, referral, reporting, and maintenance of collateral contacts. The Recovery Services Treatment Coordinator regularly communicates, collaborates, and coordinates with the Primary Counselors, Clinical Supervisor, Peer Recovery Specialists, and Intake Supervisor, to obtain necessary direct coordination service support. This position requires self-confidence, creativity, and an ability to exercise individual discretion or judgment. Sound organizational skills and the ability to handle multiple tasks are a must. S/he must be able to devote the time and energy required to ensuring sessions are complete, positive, and reinforcing to clients, support agencies, and the community. S/he must possess a high level of crisis tolerance, and the ability to work through crisis situations in a professional, therapeutic, and positive Manner. In addition to assisting coordination of client needs, this position oversees the WRS Grant. This position will require Grant reporting, Quartey Grant meeting attendance, Data collection and all WRS Grant assessments done on all female clients of the women’s recovery program. Pay ranges from $23-$25 per hour

Requirements

  • A bachelor’s degree in behavioral sciences or related field
  • Excellent oral and written communication skills.
  • Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success
  • Familiar with Microsoft Office applications
  • Experience using electronic health record systems
  • Current Tuberculosis screening
  • Must be able to favorably pass applicable criminal history/background checks.
  • Two years free of any problems associated with chemical dependency. Employees directly responsible for persons being served by the program are prohibited from abusing prescription medication or being in any manner under the influence of a chemical that impairs the individual’s ability to provide services or care.

Responsibilities

  • Assist in implementation treatment goal plans with each assigned client.
  • Develop and maintain effective working relationships with community service providers across the Continuum of care needed by women and children involved in treatment programming.
  • Assist clients in securing daycare/PICA and school transportation.
  • Assist and support recovery services through recovery activities.
  • Assist and support counselors in treatment planning, behavior management concerns and referrals for specialized services as needed.
  • Assist in accessing mental health services for clients and their children.
  • Provide individual treatment coordination services.
  • Maintain contacts and attend meetings with collaborating agencies toward meeting client goals and progress.
  • Monitor client activities and work assignments.
  • Maintain treatment files on assigned clients/caseload.
  • Maintain client records and supervises data collection and reporting for the WRS DHS Grant.
  • Prepare and submit required reports to DHS.
  • Attend and participate in weekly and quarterly staff meetings and trainings.
