About The Position

The Recovery Peer Specialist will function as a role model, exhibiting competency in personal recovery and use of coping skills, serving as an advocate providing peer support and resources to participants served. This includes developing person-centered recovery goals that enable individuals to live, learn, socialize, work and thrive in the community. The Peer Specialist will also drive transporting staff to community designations and transporting participants to various services, in addition to driving for agency business. Requirements

  • Certification as a Certified Recovery Peer Advocate or CRPA-P working on completion of certification.
  • High School Diploma or its equivalent with 2-3 years passenger van/mobile unit driving experience and clean driving record.
  • 5-7 years passenger van/mobile unit driving experience with a clean driving record.
  • Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be substance free.
  • Friendly, courteous, service-orientated, professional, outgoing, and client service.
  • Must be able to work independently and productively with minimum supervision.
  • Team player with a “can do” attitude that can work in a fast paced environment.
  • Ability to establish and maintain professional atmosphere for co-workers and clients.
  • Organizes tasks, manages time well and directs oneself.
  • Performs job tasks in a manner that does not result in negative performance reviews.
  • Performs duties in a manner that expedites work requirements.
  • Manages time, adheres to deadlines and prioritizes tasks.

Responsibilities

  • Provide outreach to individuals with a history of Substance Use whose behaviors put them at risk, in order to facilitate their engagement in services and offer additional supports.
  • Assists with the development of wellness recovery action plans and recovery plan.
  • Assists participants with identifying sustaining coping techniques and self-help strategies.
  • Provides health coaching and recovery support, especially on issues of self-advocacy, wellness self-management, medication adherence, entitlements, and daily living activities.
  • Life skills or support group facilitation.
  • Picks up newly discharged participants from various locations and referral sources.
  • Accompanies participants as needed to various appointments.
  • Encourages family involvement when appropriate and provide family education and support.
  • Provides telephonic as well as face to face outreach, engagement and service planning in the community.
  • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
  • Acts as a resource to all team members on team planning activities around substance abuse/MATS issues.
  • Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedures.
  • Accesses records only where the “need to know” exists.
  • Follows established Federal, State and Elmcor guidelines on confidentiality.
  • Assists participants with their case manager in determining steps necessary to achieve treatment goals.
  • Assists Case Manager to support participants, as needed, during interventions.
  • Recovery education for community members.
  • Post-treatment monitoring and support.
  • Provide recovery check-ups and coaching.
  • Seeks supervision and consultation as needed.
  • Transportation of staff within the community to designated venues to provide services.
  • Transportation of staff conducting agency business.
  • Transportation of prospective participants to needed services.
