About The Position

Join the team as a Remote Care Manager focusing on the Engagement and Recovery Initiative. In this role, you serve as the essential link between various treatment components, ensuring that adult and child clients are supported from their initial intake through their long-term recovery journey. This position utilizes a strength-based model to provide comprehensive care coordination and transition support.

Requirements

• Bachelor’s degree in a health-related behavioral science field (Psychology, Social Work, etc.).
• Strong communication, active listening, and the ability to collaborate effectively in a remote team environment.
• A commitment to treating every individual with respect and focusing on the unique strengths of families and individuals.

Responsibilities

• Conduct timely post-intake follow-up calls to ensure a positive experience and explain the value of care management services.
• Facilitate initial screenings, assist in developing person-centered care plans, and secure necessary consents for service coordination.
• Act as a liaison between physical and behavioral health providers, helping clients schedule appointments and overcoming barriers to treatment compliance.
• Maintain regular weekly contact with clients to reinforce engagement, monitor progress, and ensure services meet the client's identified needs.
• Facilitate the successful transition of clients between initiative teams and local community centers, ensuring continuity of care.
• Connect clients with essential community resources such as housing, food, and employment, while providing education on independent living skills.
