About The Position

High Point & Affiliated Organizations is a health and human service agency dedicated to treating and preventing substance use disorders and mental illness. We offer a comprehensive continuum of care across Southeastern Massachusetts, including inpatient, outpatient, residential, and community-based services. Our programs also support survivors of abuse, violence, and families experiencing homelessness. We believe in the inherent goodness, worth, and dignity of every individual and aim to help people and families achieve personal change and improve their quality of life. Our outpatient services in Brockton, New Bedford, and Plymouth provide individual, group, and family services, as well as medication management, with options for day and evening appointments. Our interdisciplinary team includes licensed social workers, clinicians, psychiatrists, and psychologists.

Requirements

Must present as courteous and professional at all times.
Demonstrated flexibility and ability to perform multiple tasks.
Excellent verbal and written communication skills, including the ability to organize ideas logically and clearly.
Proficient and knowledgeable in basic computer skills and software programs, including the ability to learn, use, and train HPAO's electronic medical records.
Understanding of the relationship between substance use and mental health issues and/or self-help programs.
Knowledge of the addiction treatment services system and recovery support services.
Understanding of boundaries and ethics.
Compliance with Federal Regulations, 42 CFR Part 2, Substance Abuse Confidentiality Regulations, and HIPAA Regulations.
Certification in the HPAO approved de-escalation program.
Ability to work collaboratively with alcohol/drug dependent clients from diverse legal, social, economic, ethnic, cultural, and racial backgrounds, as well as family members, insurance providers, and outside providers.
Ability to work independently while adhering to the program’s philosophical team approach.
Established contacts and recovery partnerships in the community.
Participation in annual TB screening.
CPR Certification.
High school diploma.
Lived experience of substance use and recovery.
Valid driver’s license.
Clean driving record.
Ownership of a car.

Responsibilities

Provide emotional and social support to Members.
Support Members in making positive life changes and developing skills for recovery.
Help Members explore and implement new strategies for building recovery-supportive friendships, improving family relationships, and utilizing recovery-community networks.
Assist Members in connecting with treatment, peer recovery support services, and mutual aid groups, and support their ability to navigate these services.
Use the peer relationship to assist with motivation and facilitate connections to primary and specialty medical, dental, and mental health services, as well as social services, including benefit applications and system navigation (e.g., criminal justice, child protection).
Act as a recovery liaison and support Members in preparing for or accompanying them to meetings with probation officers, social workers, and child protection/child welfare workers.
Provide linguistically appropriate and culturally sensitive peer recovery supports that respect the diversity of Members' identities (racial, ethnic, gender/gender identity, sex, sexual orientation, physical and intellectual challenges, and chosen recovery pathway).
Serve as an advocate for Members and assist them in developing self-advocacy skills.
Act as a mentor, supporting the Member's recovery process, goals, and decisions, and assist in creating and enacting a Wellness Plan.
Provide temporary transportation assistance to essential self-help, peer support, and medical/behavioral health appointments while transitioning to community-based resources.
Deliver services on a mobile basis in any setting safe for the Member and staff.
Collaborate with pregnant and/or parenting Members to create and coordinate a Plan of Safe Care (or Family Support Plan) to identify needed recovery and parenting services.
Support Members with perinatal health, housing, healthcare, income, and mental health needs.
Establish plans for follow-up and address barriers to treatment, preventative care appointments, vaccine access, and reproductive healthcare screenings.
Participate in community events promoting prevention and education.
Work with the NCM/Patient resource navigator to coordinate community referrals.
Assist patients requiring additional supports and service coordination for engagement.
Be available at least once per week to support families and individuals struggling with SUD in underserved and disenfranchised populations.
Facilitate peer groups in Inpatient settings to increase access to services and promote a collaborative treatment approach.
Support patient engagement, including housing, appointment coordination, and transportation.
Document patient engagements in the EHR system.
Maintain appropriate productivity/billable hours.
Become certified within 1 year of employment.
Perform other duties as assigned by the Recovery Coach Supervisor/Program Director.
