The Re-engagement Specialist is responsible for conducting in-person outreach to clients who have been disengaged from services for more than thirty days. The primary objective of this role is to re-engage these clients and encourage them to resume utilizing the services provided by Preventive Measures. The Re-engagement Specialist will work closely with the client, providing support, information, and assistance to address any barriers preventing re-engagement. This position requires excellent communication skills, empathy, and a proactive approach to problem-solving. The role also involves maintaining positive, cooperative, and mutually supportive relationships with supervisors, administrative, peer, and support staff, and establishing a professional rapport with clients. Demonstrating initiative, enthusiasm, and positive interpersonal skills to relate well with clients and staff is essential.
