Reporting to the Director of Care, the RAI Coordinator is a critical role within our communities as it impacts the lives of residents.

Requirements

  • Current general certificate of registration with a college of a regulated health profession or Ontario College of Social Workers with a minimum of three years clinical and project experience.
  • Has a valid license to practice.
  • Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC.
  • Good problem solving and analytical skills.
  • Must be legally eligible to work in Canada or have a valid work/study permit.
  • Must successfully pass a Vulnerable Sector Check and/or Criminal Record Check applicable to the appropriate province as well as a tuberculosis screening.

Responsibilities

  • Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
  • Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
  • Review and analyze each resident’s RAI assessments to ensure accuracy and meet the residents needs.
  • Monitor and analyzes RAI outputs and reports.
  • Produce RAI reports for leadership use to assist in clinical and operational decision-making.
  • Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC). Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.
  • Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
  • Lead the RAI education process for all participating team members.
