About The Position

The Quality Reviewer supports VNATC Home Health in managing quality of care, utilization, documentation, outcomes, and processes through associate interaction, education, and timely review of electronic patient records. The position integrates and demonstrates VNA core values in all work responsibilities.

Requirements

• At least one year of recent Home Health experience related to OASIS coding and review.
• Knowledge of CMS Home Health regulations, including current Conditions of Participation (COPs).
• Active RN or PT license in the State of Florida.
• Current Basic Life Support for Health Care Provider certification/CPR.
• Must undergo screening through the Care Provider Background Screening Clearinghouse.

Nice To Haves

• Knowledge of or experience with ICD-10 coding.
• Experience with Homecare Homebase EMR.

Responsibilities

• Review and approve patient information and clinician documentation provided by the assessing clinician at OASIS timepoints, including SOC, ROC, recertification, evaluation visits, and discharge.
• Analyze OASIS data to ensure accuracy.
• Review diagnosis coding and plans of care (POC/485) for accuracy and completeness.
• Ensure patients are appropriate for home care requirements and that applicable payer rules and regulations are followed.
• Communicate regularly and effectively with assessing clinicians and clinical managers to facilitate care planning throughout each client’s admission.
• Identify high-risk cases requiring interdisciplinary case conferencing.
• Review clinical records for accuracy and completeness on an ongoing basis.
• Collaborate with the QAPI nurse and home health educator regarding review findings and training needs.
• Participate in the QAPI Committee.
• Perform other duties as assigned.
