About The Position

Muskegon Family Care is looking for a Full- Time Quality Improvement Specialist to join our Supportive & Collaborative Team. Requirements

  • Knowledge of medical terminology and medical abbreviation.
  • High school diploma or GED (Required).
  • CPR certification required within 6 months of employment.

Nice To Haves

  • Registered or Certified Medical Assistant preferred.
  • Three to six months’ related experience and/or training preferred.
  • Computer and Medical Terminology experience preferred.

Responsibilities

  • Review charts for missed services and schedule patients for appointments.
  • Assembles gift bags and fit kits
  • Pull UDS Measure reports and contact patients to schedule appointments utilizing Azara registry.
  • Contacts patients to attend smoking cessation classes.
  • Fit Kit pick up- contact patients to complete fit kit and drive to collect completed fit kits.
  • Works missed services reports received from QI Analyst.
  • Answers telephone and reply to voicemail messages.
  • Enter data into patient electronic medical records when necessary.
  • Orders due to preventive services when necessary.
  • Maintains a courteous, helpful and conscientious attitude in dealing with co-workers and patients and their families, always ensuring professionalism and confidentiality.
  • Understands all aspects of Healthcare Effectiveness Data and Information Set (HEDIS) scores and reporting.
  • Understand and implement Patient Centered Medical Home initiatives to increase quality of care and provider reimbursement from BCBSM.
  • Understands and implements healthy People 2020 goals and initiatives to improve measures.
  • Regular and prompt attendance at work.
  • All other duties assigned.
