About The Position

This role involves leading and coordinating the organization's participation in value-based reimbursement programs, monitoring patient care, analyzing data, and collaborating with various teams to improve outcomes and organizational performance. The coordinator will also support regulatory and quality improvement initiatives.

Requirements

  • Minimum of Associate Degree in Nursing required.
  • Minimum of two (2) years of administrative experience or equivalent in a medical or hospital setting.
  • Minimum of five (5) years in a clinical setting required.
  • Active RN license in the state of SC.

Nice To Haves

  • Bachelor's Degree in healthcare related field preferred.
  • Prior experience in quality, regulatory, and/or compliance highly preferred.

Responsibilities

  • Lead and coordinate the organization's participation in the CMS TEAM model and other value-based reimbursement programs, ensuring performance goals, reporting requirements, and financial targets are met.
  • Monitor patient episodes across the continuum of care, identify opportunities to improve outcomes and reduce avoidable costs, and work with clinical teams to address gaps in care.
  • Build strong relationships with physicians, advanced practice providers, and operational leaders to drive engagement, accountability, and performance improvement efforts.
  • Analyze clinical, operational, quality, and financial data to identify trends, risks, and opportunities that impact patient outcomes, reimbursement, and organizational performance.
  • Collaborate with external consultants, payer representatives, and internal stakeholders to implement best practices and support ongoing program success.
  • Develop and maintain dashboards, scorecards, reports, and presentations that provide actionable insights for leaders, committees, and care teams.
  • Partner with Case Management, Revenue Cycle, Clinical Informatics, Information Technology, and other departments to optimize workflows, documentation, and reporting processes.
  • Support regulatory, accreditation, quality improvement, and patient safety initiatives through data collection, analysis, education, and performance monitoring.
  • Participate in organizational quality initiatives and special projects, performing additional quality and performance improvement duties as assigned.
