The Health Home Quality & Education Director is responsible for overseeing quality improvement and compliance initiatives for the Health Home program. This position provides leadership for the Health Home quality team and the Health Home Specialized Health Consultants and ensures the program’s policies and procedures meet organizational guidelines, funder requirements and accreditation standards. This position fulfills the Health Home Director role per DMH Health Home guidelines. The role involves creating and revising Health Home policies and procedures, establishing compliance structure and oversight, coordinating accreditation reviews and funder audits, and providing input on program strategic initiatives. It also includes overseeing program data management systems, managing quality-related reporting, tracking and trending quality data, and ensuring data consistency with program objectives. The Director oversees the quality enhancement process for internal program audits, provides training topic information to leadership, manages the Health Home quality team (including hiring, training, coaching, and development), supervises Health Home Specialized Health Consultants, attends DMH Health Home meetings, serves on the Senior Leadership Team, and participates in budget creation and financial monitoring for the quality team.
