Quality & Education Director - Health Homes | Starting at $70,000 / year
About The Position
The Health Home Quality & Education Director is responsible for overseeing quality improvement and compliance initiatives for the Health Home program. This position provides leadership for the Health Home quality team and the Health Home Specialized Health Consultants and ensures the program’s policies and procedures meet organizational guidelines, funder requirements and accreditation standards. This position fulfills the Health Home Director role per DMH Health Home guidelines. The role involves creating and revising Health Home policies and procedures, establishing compliance structure and oversight, coordinating accreditation reviews and funder audits, and providing input on program strategic initiatives. It also includes overseeing program data management systems, managing quality-related reporting, tracking and trending quality data, and ensuring data consistency with program objectives. The Director oversees the quality enhancement process for internal program audits, provides training topic information to leadership, manages the Health Home quality team (including hiring, training, coaching, and development), supervises Health Home Specialized Health Consultants, attends DMH Health Home meetings, serves on the Senior Leadership Team, and participates in budget creation and financial monitoring for the quality team.
Requirements

- Intermediate-level proficiency with Microsoft Office products, including Excel.
- Knowledge of Electronic Health Record systems.
- Must have bachelor’s degree in human services field (social work, psychology, counseling, etc.).
- Must have at least 3 years of management experience in IDD, behavioral health or medical systems.
- Must have experience using Microsoft Office suite.
- Must have intermediate-level skill with Excel.
- Must have current driver’s license and proof of insurance.
- Must complete and maintain all required trainings/certifications.
- Must maintain confidential information in accordance with HIPAA regulations.

Responsibilities

- Coordinates the tracking and trending of Health Home quality data.
- Ensures Health Home data and metrics are consistent with program, funder and organizational objectives.
- Oversees the quality enhancement process for internal program audits and reports, including action plans to ensure timely resolution for identified concerns.
- Provides information to leadership regarding additional training topics based on identified trends and ongoing regulatory requirements.
- Manages the Health Home quality team in carrying out program activities.
- Ensures staffing ratios are met and that team objectives are accomplished in an effective and timely manner. This includes hiring, training, coaching and developing team members.
- Supervises Health Home Specialized Health Consultants, including hiring, coaching and developing team members.
- Ensures team objectives are accomplished and that specialized trainings are effective through evaluating attendance, pre/post-assessments and surveys.
- Attends and contributes to DMH Health Home meetings to improve processes and services.
- Serves as a member of the Senior Leadership Team and participates in committees and ad hoc projects as needed.
- Participates in the creation of the program’s annual budget.
- Responsible for monitoring and making adjustments related to monthly financials for the quality team.
