About The Position

Provides administrative support activities to the Quality department. Under direct supervision of the Quality Assurance Manager, responsible for performing a variety of clinical support functions for the outpatient setting including patient outreach, care gap analyses, and patient follow-up. Maintains documents and records according to the state and federal health (HIPAA) care standards.

Requirements

  • High School graduate or equivalent required
  • Computer literacy with extensive knowledge of spreadsheets and graphs
  • Must have reliable transportation to perform clinical site audits within Bexar, Kendall and Hays counties

Nice To Haves

  • Quality Improvement Specialist or Medical Assistant experience preferred
  • Bilingual (English/Spanish) is preferred

Responsibilities

  • Researches and complies information in a timely and effective manner to assist in the preparation, dissemination, and presentation of the program and annual evaluation of the clinical Quality Performance Improvement Plan activities.
  • Utilizes quality care model Plan, Do, Study Act (PDSA) implement, coordinate, and monitor all programs and activities through the direction of the Quality Assurance Manager.
  • Assist the Quality staff to collect, track and perform internal and external clinical audits and/or analyzes data reports.
  • Assist the Manager to prepare and implement processes to achieve improved performance.
  • Gathers, enters, and/or updates data to maintain departmental records.
  • Accurate and timely data entry into databases as appropriate; establishes and maintains files and records for the program.
  • Present summary reports and trend data analyses of program activities on a regularly scheduled basis to the Quality Assurance Manager.
  • Provides excellent internal/external customer service via telephone, fax or face-to-face contact to assist staff and patients with their health care needs and services as needed.
  • Serves as a primary point and liaison with departments in generating and/or coordinating the development of various reports regarding performance and quality outcomes and prepares responses as appropriate with the approval of the Quality Assurance Manager.
  • Identifies and follows up on care gaps in care that cannot or were not closed in the clinic.
  • Performs patient outreach to schedule Post ED Visits and Admissions and Follow up appointments to fill gaps in care.
  • Performs other related duties as assigned.
