About The Position

Under the direction of the Prime Health Program Manager, the Prime Health Coordinator supports the day-to-day operation and growth of CommuniCare's employer health program. The position coordinates employer-group onboarding, member engagement, events, communications, documentation, and reporting while helping ensure a consistent, responsive, concierge-level experience for employer partners and Prime Health members.

Requirements

  • Bachelor’s degree in Public Health, Health Science, Social Work, Health Education, or a related field required
  • Minimum of two (2) years of experience in community outreach, clinical experience, public health, or health equity initiatives
  • Strong understanding of customer service and client needs assessments, with the ability to analyze data and use it to drive decision-making
  • Proficiency in Microsoft Office software.

  • Bilingual English/Spanish skills are preferred

Responsibilities

  • Assist with maintaining strong relationships with organizational leaders and stakeholders in maintaining a consistent, concierge-level experience across all employer and member-facing communications.
  • Coordinate onboarding and renewal activities for employer groups, including milestones, required information, communications, internal readiness, and follow-up.
  • Coordinate logistics for employer events, mobile health unit visits, health fairs, screenings, workshops, and other member-engagement activities.
  • Perform outreach efforts to raise awareness about the health services provided to employer partner groups.
  • Collaborate with clinical, operations, marketing, population health, billing, information technology, and other departments to support employer and member needs.
  • Assist Prime Health Program Manager as needed to create and develop educational materials, toolkits, and resources for partners and health professionals.
  • Ensure that health services are culturally and linguistically appropriate for diverse populations.
  • Maintain accurate employer-group records, eligibility references, onboarding documents, standard operating procedures, and other program materials.
  • Prepare and maintain employer-facing materials, toolkits, presentations, and communications consistent with established CommuniCare branding standards.
  • Track onboarding milestones for new and renewing employer groups.
  • Support data entry and reporting related to employer group performance, member engagement, and utilization, as requested by the Program Manager.
  • Monitor routine employer, broker, and member inquiries; coordinate timely resolution and escalate significant service, clinical, or operational concerns as appropriate.
  • Help maintain internal reference materials, such as employer group eligibility tables and onboarding SOPs, keeping them current as plan details change.
  • Performs other related duties as assigned.
