About The Position

Pathways is designated as a Mental Health Rehabilitation/Targeted Case Management (MHR/TCM) Provider Center of Excellence by Superior HealthPlan for our Mosaic Behavioral Health program. The Qualified Mental Health Professional (QMHP) for Pathways Treatment Family Foster Care (PTFFC) provides ongoing monitoring, support and behavioral health support services to children and youth with emotional trauma as well as to serve their families. The focus population of this position is children and adolescents with high acuity behavioral health challenges. The QMHP will engage the children and family minimally in weekly skills training and case management (routine and intensive) service. The QMHP will follow a strengths-based model to develop and build skills that will help the children, youth and families function autonomously and successfully. This position will work with the clinical team to develop a Service/Treatment Plan focused on the child and family’s strengths and needs.

Requirements

Bachelor’s Degree from an accredited college or university and a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
At least two years of experience working directly with children, families, and other individuals with complex behavioral health issues.
Completes and attests to completion of all mandatory training as identified in HHSC UMCM chapter 15.3.

Proof of valid Texas Driver's License (Type C) and at least three years of driving experience.

Three professional employment references.
Cleared criminal background check and signed statement regarding felony indictments/convictions.
Cleared TB test results (current within 12 months prior to employment).
Cleared pre-employment drug screen.
Working cellular telephone.

Responsibilities

Enters demographic data, contacts made, and other required data for all newly assigned clients into the electronic record system in a timely manner (i.e., 24 hours).
In consonance with the PTFFC Behavioral Therapist, when applicable, facilitates the development of an individualized Treatment Plan within seven days of intake when the assessment is completed.
Works with child, youth and family teams to develop a Crisis Plan to ensure the safety of the child, family, and community.
Utilizes approved skills curriculum with children, youth and families to help improve behavior.
Provides weekly billable services that effectively support the client’s LOC and Service/Treatment plan goals.
Provides services in consonance with the TBRI-Caregiver and Together Facing the Challenge models, supporting other treatment team members in reinforcing these models.
Provides support and crisis management services as needed or as requested by the supervisor.
Participates in ongoing case staffing and monthly service planning and treatment team meetings with other service providers; keeping the team abreast of progress and ongoing needs.
Meets face-to-face with clients in their home, school, office and community for the provision of all services. Virtual services may be provided on a case-by-case basis.
Initially assesses and reassesses (every 90 days or sooner if required for reassessment) the needs of the client by completing a CANS/ANSA Assessment.
Meets with the client, family and all team members including internal and external Behavioral Health Therapists to modify the Treatment Plan to reflect the changes and/or progress made.
Motivates and supports family involvement that may continue post placement in the PTFFC program.
Assists client and family in obtaining any financial assistance or services for which they may be eligible under federal, state, and local regulations.
Maintains accurate and concise documentation of all services, contacts and collaboration with client, family and community partners. All documentation is submitted to the supervisor within 24 hours after service has been delivered.
Maintains an empathic, yet professional, relationship with child, youth and family and strives to keep clients and families engaged in all services.
Maintains all client documentation in the electronic record system and ensures adherence to requirements set forth in all state and federal rules and regulations.
Facilitates and/or participates in monthly foster parent training/resource groups.
Responsible for maintaining one week of on call crisis readiness on a rotating basis.
