About The Position

The Intensive Services QMHP-CS, under the supervision of Intensive Services Clinical Supervisor, performs outpatient routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care models. Provides community-based person directed case management to assist in navigating access to medical, social, vocational, financial, educational, legal, and other individualized services. Is responsible for initiating and collaboratively completing uniform assessments, then organizing and documenting the plan for services to meet the individual’s identified needs in the community, home, legal system, and school. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Position might work closely with criminal justice agencies. The goal is to link involved individuals with appropriate mental health and, potentially, substances use disorder services to reduce recidivism and inpatient hospitalizations while treating serious mental health concerns to ensure individuals receive the services that they need to remain stable in the community. Other duties as assigned to meet program and individual needs. This position practices good stewardship of resources by ensuring all required training is completed prior to due date and center-issued equipment is maintained in good condition. The Intensive Services QMHP-CS will link individuals to additional resources needed to support their goals. Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, and Lifepath Systems policies and procedures. This position will work closely with fidelity managers and leadership to ensure that all gaps are reconciled. This position builds positive relationships with community partners, and mental health professionals in efforts to increase referrals from community stakeholders and partners. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities might be required. Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve. This is a community-based position. Travel by car up to 80% may be necessary to meet caseload needs. Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record. At least 90% of service notes are signed within 2 business days of service. All full chart reviews receive a score of 80% or higher following the Golden Thread. Provides the first service within 2 weeks of receipt of the service referral. Ensures target caseload meets minimum 30, averaging 6hrs of service provision. Updates individualized treatment plans as needed. Maintains data collection and reporting standards. If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Requirements

  • Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
  • At least two years of related experience and/or training in the field of mental health with jail diversion/OCR preferred.
  • Knowledge of Trauma-Informed Care Practices required.
  • Must have reliable transportation and a valid driver’s license.
  • Knowledge and experience in providing outpatient case management, skills training, and symptom management services to individuals with mental illness, preferred.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to work independently and collaboratively within a team, taking initiative and utilizing judgment appropriately.
  • Timely and thorough documentation, time management and organizational skills, effective scheduling, ability to efficiently manage workflow.
  • Ability to respond to individuals with patience, empathy, and understanding.
  • Ability to effectively communicate regarding behavioral health needs and services.
  • Ability to connect individuals with appropriate services to support their goals, providing appropriate continuity of care.
  • Knowledge of UM Guidelines, Texas Administrative Code, and Info Item V Standards.
  • Possess the ability to think clearly and logically, to deal with potentially dangerous and/or emotional situations and utilize de-escalation techniques.

Nice To Haves

  • Jail diversion/OCR experience preferred.
  • Previous work experience, partnership, or collaboration with court and justice systems highly desired.
