SPECIFIC RESPONSIBILITIES During team meetings, advocate for the Individual's rights and needs regarding the assigned caseload. Serves as a source of contact between families/advocates/day program and residential. Works with families/advocates of the Individuals by reporting to them the Individual's progress and concerns, arranging visits, and providing schedules of meetings (ICF). Maintains current and up to date advocate/family/Care Management/Day Program information. Communicates any benefit concerns, such as entitlements, legal, Medicaid, social security, SED, OPWDD, and the public school system/day program (ICF). Secures annual consents for history books and/or SDMC paperwork for procedures (ICF). Attends all Semi/Annual/admission/discharge meetings occurring in both hospitals/day programs. Completes annual assessments to develop individual CFA during annual/semi-reviews (ICF) & SAP/IPOP (IRA) during annual/semi-reviews. Coordinate and/or update the Individual's Developmental Disabilities Profile 2. Ensure that the Level Care Eligibility Determination form is accurately completed current and/or Utilization Reviews are completed annually. Input and update plans into the electronic health record system. Analyze the data based on the execution of plans to monitor progress or regression. Participates as a member of the Interdisciplinary Treatment Team: a) participates in the planning and b) implementation of treatment plans c) including the training of staff d) and monitoring of said plans. Organize information for Special IDT, if necessary, from hospitals, programs, behavioral, dayhab & family. Coordinates with Management for any training session (when needed) with the various in-house clinicians. Assists Management with the development of program activities per the individual program plans. Participates in observation and in-service training for the implementation of goals. Ensures clinical plans of care are incorporated into program plans CFA/IPOP (OT, PT, SP, T). Obtains input and background from individuals, clinical staff, direct care staff, and Management to oversee development of program plans. Participate and collaborate on screenings and intake of potential new admissions.
