About The Position

The Pulmonary Disease Navigator is a Registered Respiratory Therapist (RRT) responsible for working directly with patients diagnosed with chronic pulmonary diseases such as COPD, emphysema, pneumonia, asthma, and pulmonary fibrosis. This role acts as a liaison to patients, families, and providers to ensure proper length of stay, adequate education, and safe discharge. The Navigator ensures quality metrics specific to disease processes are met, promotes compliance with chronic pulmonary disease management throughout the continuum of care, and provides referrals for follow-up testing or to other disciplines and specialties as appropriate. The role involves completing competency assessments, utilizing experience with pulmonary function testing and GOLD Standards for COPD diagnosis, and performing various respiratory care services including medical gas delivery, pulse oximetry, administration of respiratory therapies, and arterial blood gas sampling. The Navigator also supports staff orientation and in-services, coordinates follow-up with patients, and holds monthly COPD support group meetings.

Requirements

  • Registered Respiratory Therapist (RRT)
  • Experience with pulmonary function testing
  • Experience with GOLD Standards used in diagnosing COPD
  • Experience with six minute walk
  • Current practice in area of responsibility
  • Knowledge of computer/information systems and equipment used in performing assigned duties
  • Knowledge of safety and infection control policies and procedures
  • Verbal and written communication skills that are sensitive to diversities among age groups, cultures, and education levels facilitating teaching.

Responsibilities

  • Act as a liaison to patients, families, and providers to ensure proper length of stay, adequate education, and safe discharge.
  • Ensure that quality metrics specific to the disease process are met.
  • Promote compliance with management of chronic pulmonary diseases throughout the continuum of care.
  • Provide referrals for follow-up testing and/or to other disciplines and specialties when appropriate.
  • Successfully complete a generic and department-specific competency assessment.
  • Ensure that the patient has received skilled and competent delivery of Respiratory Care Services, as ordered by a physician in accordance with established protocols.
  • Deliver medical gases, perform pulse oximetry for oxygen titration, administer and monitor respiratory and therapeutic modalities including aerosolized medications, metered dose inhalers (MDI), sputum inductions, and arterial blood gas sampling and analyses.
  • Support staff orientation, in-services, and competency needs regarding pulmonary diseases.
  • Coordinate follow-up with patients.
  • Hold monthly COPD support group meetings to ensure we are meeting the needs of our patients both in and out of the hospital.
