In support of the FY22 Choice Neighborhood Implementation Grant, this position will organize and direct the delivery of wellness, behavioral health, and care navigation services for diverse populations living in low-income and affordable housing, including adults, children, individuals with disabilities, and adults diagnosed with serious mental illness. This role focuses on connecting clients with essential local medical, health and wellness resources, coordinating wellness programming at multiple public housing sites, and facilitating access to preventive and supportive services. Additional duties include partnering with City of Tucson Housing and Community Development Resident Services team to coordinate and facilitate participation in healthy lifestyle programming and connect individuals to community resources such as food banks and food security supports. By assisting clients in completing their care plan goals, applicant will help improve individual health outcomes and strengthen community wellness. This is a grant-funded position ending September 2027.
