About The Position

The Community Health Coordinator uses an integrative approach to promote wellness and address the medical, behavioral health, substance use, and social determinants of health of an assigned panel of patients. Functions as a member of the multidisciplinary treatment team to assess needs and develop, implement, and revise comprehensive care plans for children, adults, and families. Coordinates with the care team and other community resources and supports to optimize outcomes.

Requirements

  • Bachelor's degree required.
  • Strong communication, customer service, problem-solving, and time management skills.
  • Valid Tennessee driver's license required.

Nice To Haves

  • 1-2 years of experience connecting individuals with community resources preferred.
  • Experience with Microsoft Office and electronic medical records preferred.
  • Background in nursing, social work, teaching, or related health fields; additional experience in addiction counseling or nutrition is a plus.

Responsibilities

  • Conduct outreach in homes, schools, clinics, and community settings to assess patient needs.
  • Develop and coordinate individualized care plans for children, adults, and families.
  • Connect patients with community resources, supports, and healthcare services.
  • Provide health education and encourage preventive care and chronic disease management.
  • Collaborate with providers, schools, agencies, and multidisciplinary care teams.
  • Document care coordination activities and patient progress in the electronic health record.
  • Participate in team meetings, quality improvement initiatives, and ongoing training.
  • Occasional transportation of patients to appointments using a River Valley Health vehicle.
