About The Position

This position will directly supervise a team of public health nurses and Care Coordination Assistants that provide home visiting services to clients enrolled in the DCY Parents as Teachers Home Visiting Program & Healthy Start home visiting program and ensure compliance and fidelity to evidence-based practices. This position will also assist in writing reports as well as provide some in-home family-centered assessment, education and coordination services to pregnant and parenting women, and their children up to 3 years of age with the goal to mitigate health disparities, improve birth outcomes, prepare children for school and better the health status of women, infants and children.

Requirements

  • Possession of a valid State of Ohio license as a Registered Nurse
  • Three (3) years of professional nursing experience
  • Must be willing to work with culturally diverse populations

Nice To Haves

  • A bachelor’s degree in nursing with 3 years of relevant experience in home visiting, assessment of pregnant and postpartum mothers and newborns, and staff supervision preferred
  • A master's degree in nursing or public health OR certification in a nursing specialty area may be substituted for one (1) year of the required experience
  • Some positions may require possession of a valid driver’s license

Responsibilities

  • Plans assignments of professional and non-professional nursing and other health personnel
  • Interprets agency programs and policies to staff
  • Confers with staff regarding nursing or program problems and procedures
  • Ensures that nursing staff utilize evidence-based best practices as appropriate
  • Participates in agency health conferences, case conferences, and committees with other community organizations
  • Supervises student, practical, and registered nurses and other health personnel to include signing performance evaluations, approving leave requests, and recommending disciplinary actions
  • Reviews reports and records of staff
  • Evaluates staff performance
  • Leads and participates in staff meetings
  • Maintains patient medical records and reports
  • Writes reports
  • Participates in policy development and the overall planning, implementation, and evaluation of programs and team functions
  • Coordinates patient care to reach desired outcomes
  • Acts as consultant to others
  • May perform grant management activities such as writing, monitoring, and evaluating grants and writing grant reports
  • May be assigned to serve as a member of an Incident Command System (ICS) Team, or a similar public health response team which may include the conduct of operations on a 24/7 basis at remote locations
  • Assignment may include serving as a department and/or community incident commander or on the incident command team
