Requirements

  • Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification
  • Associate's degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification

Nice To Haves

  • Experience in public health nursing, community health, primary care, women's health, immunizations, communicable disease, HIV/AIDS services, family planning, or related clinical programs.
  • Experience providing health education and case management services.
  • Bilingual skills (English/Spanish) preferred.
  • Experience working with electronic health records and Microsoft Office applications.
  • Knowledge of public health principles, disease prevention, and population health practices.

Responsibilities

  • Provides professional nursing care in accordance with established nursing protocols, policies, and procedures.
  • Focuses on disease prevention, health promotion, care coordination, and maintenance of the health of individuals, families, and populations.
  • Performs clinical assessments, develops plans of care, provides treatment and health education, coordinates services, and collaborates with healthcare providers and community partners to improve health outcomes.
  • Performs comprehensive health assessments and evaluates client healthcare needs.
  • Develops, implements, and modifies plans of care based on assessment findings.
  • Provides treatments, medications, immunizations, and other nursing interventions in accordance with established protocols.
  • Delivers preventive health, maintenance care, and health promotion services.
  • Provides health education and counseling to individuals and families.
  • Screens clients and coordinates referrals to healthcare providers, specialists, and community resources.
  • Participates in care coordination and case management activities.
  • Maintains accurate clinical documentation, reports, and records.
  • Participates in planning, implementation, and evaluation of public health programs and services.
  • Collaborates with multidisciplinary teams and community partners.
  • Participates in emergency preparedness, disaster response, and public health emergency activities.
  • Conducts outreach activities, home visits, chart reviews, or follow-up activities as assigned.
  • Cross-trains in various public health programs and services.
  • Performs job responsibilities with minimal supervision.
  • Performs other duties as assigned
