Requirements

Bachelor's degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification
Associate's degree in nursing from an accredited college or university AND One year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification

Nice To Haves

Experience in public health nursing, community health, primary care, women's health, immunizations, communicable disease, HIV/AIDS services, family planning, or related clinical programs.
Experience providing health education and case management services.
Bilingual skills (English/Spanish) preferred.
Experience working with electronic health records and Microsoft Office applications.
Knowledge of public health principles, disease prevention, and population health practices.

Responsibilities

Provides professional nursing care in accordance with established nursing protocols, policies, and procedures.
Focuses on disease prevention, health promotion, care coordination, and maintenance of the health of individuals, families, and populations.
Performs clinical assessments, develops plans of care, provides treatment and health education, coordinates services, and collaborates with healthcare providers and community partners to improve health outcomes.
Performs comprehensive health assessments and evaluates client healthcare needs.
Develops, implements, and modifies plans of care based on assessment findings.
Provides treatments, medications, immunizations, and other nursing interventions in accordance with established protocols.
Delivers preventive health, maintenance care, and health promotion services.
Provides health education and counseling to individuals and families.
Screens clients and coordinates referrals to healthcare providers, specialists, and community resources.
Participates in care coordination and case management activities.
Maintains accurate clinical documentation, reports, and records.
Participates in planning, implementation, and evaluation of public health programs and services.
Collaborates with multidisciplinary teams and community partners.
Participates in emergency preparedness, disaster response, and public health emergency activities.
Conducts outreach activities, home visits, chart reviews, or follow-up activities as assigned.
Cross-trains in various public health programs and services.
Performs job responsibilities with minimal supervision.
Performs other duties as assigned
