About The Position

Under the general direction of the Public Health Services Supervisor, the Public Health Nurse delivers a full range of population-based public health nursing services to support the department’s programs. This position promotes health, prevents disease and injury, and protects the community through assessment, education, collaboration, and direct service. The Public Health Nurse provides services related to communicable disease, immunizations, sexual health clinic and maternal and child health. Core business hours are 8:00 AM to 4:30 PM, however occasional evenings and weekends may be needed. This position requires someone who is well organized and able to work on multiple projects.

Requirements

Baccalaureate degree in Nursing
Registration as a professional nurse in the State of Wisconsin
1-2 years of nursing experience
Possession of a valid Wisconsin driver's license
Current certificate of vehicle insurance
Access to reliable transportation to all assigned work locations
CPR certified, or able to become certified within 90 days of hire
Must meet the requirements of HSS 139.08

Responsibilities

Makes meaningful observations and assessments, develops care plans, explains and interprets public health laws, rules and regulations, and interacts effectively with patients, their families, and the general public.
Participates in trainings and educational opportunities.
Implements public health laws, regulations and policies related to specific programs.
Participates in departments quality improvement initiatives.
Communicates compassionately, effectively and professionally with patients, their families, the Department of Social Services, schools, hospitals, community agencies and the general public.
Assesses strengths of individuals and communities and responds appropriately to their needs based on sensitivity to and respect for their diverse cultural and ethnic backgrounds and socioeconomic status.
Employs strategies to assure culturally-sensitive public health and health service delivery systems.
Coordinates and collaborates with medical and other professional services affecting the health care of the patient and family.
Demonstrates knowledge of current nursing practice, public health nursing practices and programs, community resources, and inter‑professional relationships involved in implementation of programs.
Applies basic public health sciences including behavioral and social science, biostatistics, epidemiology, and prevention of chronic and infectious diseases and injuries.
Participates in on call rotation 6-7 times per year to address after hour public health call/issues.
Performs a wide variety of professional nursing functions necessary for those assignments.
Provides educational programs on a wide range of health and wellness issues.
Provides guidance and service in partnership with area schools.
Conducts appropriate follow‑up on reported communicable diseases.
Conducts public health clinics (e.g., immunization, influenza, STI).
Counsels patients and families concerning their health and social needs, and assists with implementation of care plans when applicable.
Provides in‑home nursing visits to targeted high risk populations (e.g., prenatal and postpartum women, high risk infants, special ethnic).
Tracks hours and reports to Supervisor in a time appropriate manner.
Work direction may be given to support staff.
Assigns and reviews other employees' work as a group leader.
Assists in provision of student experiences.
Assists with orientation and guidance of new staff members.
Leads and participates in community groups and coalitions to address specific community health issues.
Performs daily duties independently.
Makes decisions within areas of responsibility concerning scheduling of patients, managing care plans, determining priority of work independently.
Responds to public health emergencies.
Provides Prenatal Care Coordination (PNCC) nursing visits for pregnant clients following Medicaid regulations regardless of race, ethnicity, culture, age or language spoken.
Takes a leading role in the Health Department’s Wisconsin Childhood Lead Poisoning Prevention Program, including assessment, surveillance, outreach, education, data collection and reporting.
Provides jail visits to pregnant and/or postpartum clients.
Conducts home visits including Postpartum, Crib, weight checks and outreach providing assessment, screening and information on health, development and community resources.
Conducts pregnancy tests and makes appropriate referrals.
Provides STI (sexually transmitted infection) follow-up, testing and treatment to community.
Actively participates in population-based MCH grant activities including coalitions.
Other duties may be required and assigned.
