About The Position

Under the general direction of the Public Health Services Supervisor, the Public Health Nurse delivers a full range of population-based public health nursing services to support the department’s programs. This position promotes health, prevents disease and injury, and protects the community through assessment, education, collaboration, and direct service. The Public Health Nurse provides services related to communicable disease, immunizations, sexual health clinic and maternal and child health. Core business hours are 8:00AM to 4:30PM, however occasional evenings and weekends may be needed. This position requires someone who is well organized and able to work on multiple projects.

Requirements

Baccalaureate degree in Nursing
Registration as a professional nurse in the State of Wisconsin
1-2 years of nursing experience
Possession of a valid Wisconsin driver's license
Current certificate of vehicle insurance
Access to reliable transportation to all assigned work locations
CPR certified, or able to become certified within 90 days of hire
Must meet the requirements of HSS 139.08

Responsibilities

Make meaningful observations and assessments, develop care plans, explain and interpret public health laws, rules and regulations, and to interact effectively with patients, their families, and the general public.
Participate in trainings and educational opportunities.
Implement public health laws, regulations and policies related to specific programs.
Participate in departments quality improvement initiatives.
Communicate compassionately, effectively and professionally with patients, their families, the Department of Social Services, schools, hospitals, community agencies and the general public.
Assess strengths of individuals and communities and respond appropriately to their needs based on sensitivity to and respect for their diverse cultural and ethnic backgrounds and socioeconomic status.
Employ strategies to assure culturally-sensitive public health and health service delivery systems.
Coordinate and collaborate with medical and other professional services affecting the health care of the patient and family.
Demonstrate knowledge of current nursing practice, public health nursing practices and programs, community resources, and inter‑professional relationships involved in implementation of programs.
Apply basic public health sciences including behavioral and social science, biostatistics, epidemiology, and prevention of chronic and infectious diseases and injuries.
Participate in on call rotation 6-7 times per year to address after hour public health call/issues.
Provide educational programs on a wide range of health and wellness issues.
Provide guidance and service in partnership with area schools.
Conduct appropriate follow‑up on reported communicable diseases.
Conduct public health clinics (e.g., immunization, influenza, STI).
Counsel patients and families concerning their health and social needs, and assist with implementation of care plans when applicable.
Provide in‑home nursing visits to targeted high risk populations (e.g., prenatal and postpartum women, high risk infants, special ethnic).
Track hours and report to Supervisor in a time appropriate manner.
Work direction may be given to support staff. The incumbent may assign and review other employees' work as a group leader.
Assist in provision of student experiences.
Assist with orientation and guidance of new staff members.
Lead and participate in community groups and coalitions to address specific community health issues.
Performance of daily duties is done independently.
Make decisions within areas of responsibility concerning scheduling of patients, managing care plans, determining priority of work independently.
Respond to public health emergencies.
Provide Prenatal Care Coordination (PNCC) nursing visits for pregnant clients following Medicaid regulations regardless of race, ethnicity, culture, age or language spoken.
Take a leading role in the Health Department’s Wisconsin Childhood Lead Poisoning Prevention Program, including such duties as: assessment, surveillance, outreach, education, data collection and reporting.
Provide jail visits to pregnant and/or postpartum clients.
Conduct home visits including Postpartum, Crib, weight checks and outreach providing assessment, screening and information on health, development and community resources.
Conduct pregnancy tests and make appropriate referrals.
Provide STI (sexually transmitted infection) follow-up, testing and treatment to community.
Active participation in population-based MCH grant activities including coalitions.
Other duties may be required and assigned.
