The San Francisco Department of Public Health (SFDPH) is seeking a Public Health Nurse to perform professional nursing duties under general supervision. This role involves providing public health nursing services in maternal, child, and adult health, including conducting home visits, interpreting treatment plans, providing health education, and assessing the health, safety, and development of newborns. The nurse will also provide direct nursing services to vulnerable populations such as children, adults, and older adults with disabilities, chronic illnesses, mental health, and/or substance use disorders. Responsibilities include assisting priority populations with access to medical care, linking/referring cases to appropriate resources, maintaining case supervision, and keeping records of services rendered. The role also involves nursing services for infection control, interviewing cases, interpreting medical advice, and instructing patients. The nurse will apply community organization and collective impact strategies, perform case finding, maintain nursing documentation, and prepare statistical information and reports. Additionally, the position requires conducting comprehensive health, psychosocial, educational, and environmental assessments/screenings, providing care coordination for complex medical needs, and utilizing a race equity framework to address health inequities.
