About The Position

Under general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings. This is a Non-Merit position, meaning it is not classified within the Baltimore County Government Classification and Compensation Plan. A vacancy exists in the Department of Health and Human Services, Adult Evaluation and Review Services Program. Requirements

  • Graduation from an accredited college or university with a bachelor's degree in nursing.
  • OR Graduation from an accredited diploma school of nursing; Plus At least one year of professional nursing experience.
  • OR Graduation from an accredited college or university with an associate in Arts degree in nursing: Plus at least two years' professional nursing experience.
  • Additional qualifying education may be substituted on a year-for-year basis for the required experience.
  • Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners for Nurses, is required at the time of appointment.
  • Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Responsibilities

  • Provides nurse monitoring for the In-Home Aide Services Program
  • Provides nursing assessments, person-centered care planning, and referral services to individuals, families, groups, and institutions.
  • Reviews Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.
  • Contacts assigned clients/representatives in a timely manner.
  • Conducts home visits.
  • Completes nursing assessments and high-quality person-centered care planning in a timely manner to ensure timely access to home and community-based services.
  • Provides case management, ongoing monitoring, and evaluation of the person-centered care plan.
  • Assesses and monitors clients' physical and mental states of health and determines the need for community-based resources.
  • Ensures compliance with state, federal and Medicaid regulations.
  • Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
  • Participates in the instruction and orientation of new public health nurses.
  • Prepares and maintains nursing records and reports.
  • Performs other related duties as required.
