About The Position

Under general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings. This is a Non-Merit position, meaning it is not classified within the Baltimore County Government Classification and Compensation Plan. A vacancy exists in the Department of Health and Human Services, Adult Evaluation and Review Services Program. Requirements

Graduation from an accredited college or university with a bachelor's degree in nursing.
OR Graduation from an accredited diploma school of nursing; Plus At least one year of professional nursing experience.
OR Graduation from an accredited college or university with an associate in Arts degree in nursing: Plus at least two years' professional nursing experience.
Additional qualifying education may be substituted on a year-for-year basis for the required experience.
Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners for Nurses, is required at the time of appointment.
Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Responsibilities

Provides nurse monitoring for the In-Home Aide Services Program
Provides nursing assessments, person-centered care planning, and referral services to individuals, families, groups, and institutions.
Reviews Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.
Contacts assigned clients/representatives in a timely manner.
Conducts home visits.
Completes nursing assessments and high-quality person-centered care planning in a timely manner to ensure timely access to home and community-based services.
Provides case management, ongoing monitoring, and evaluation of the person-centered care plan.
Assesses and monitors clients' physical and mental states of health and determines the need for community-based resources.
Ensures compliance with state, federal and Medicaid regulations.
Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.
Participates in the instruction and orientation of new public health nurses.
Prepares and maintains nursing records and reports.
Performs other related duties as required.
