About The Position

Join Lorain County Public Health as a Public Health Nurse and help give every child the healthiest possible start. In this rewarding role, you'll support families with newborns and children with complex medical needs through Family Connects Ohio (FCO), the Complex Medical Help (CMH) Program, and the Lead Program. Through home visits, specialized case management, education, and care coordination, you'll empower families, connect them with essential community resources, and make a lasting impact on the health and well-being of children throughout Lorain County.

Requirements

  • Licensed Registered Nurse (RN) pursuant to Section 4723.13 of the Ohio Revised Code.
  • Associate Degree in Nursing (ADN) with a minimum of one (1) year of relevant nursing experience required.
  • Current BLS/CPR certification.
  • Successful completion of home visiting training and credentialing within three (3) months of hire.
  • Proof of up-to-date healthcare provider vaccination status (or actively in the process of completing).
  • Valid Ohio driver’s license, proof of auto insurance, and access to reliable transportation for countywide travel.
  • Ability to lift up to 40 lbs and wear required Personal Protective Equipment (PPE), including face masks and shields.
  • Proficiency with electronic documentation/EHR systems and Microsoft Office.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) with three (3) years of nursing experience.
  • Experience in community health, home visiting, pediatric care, or case management.
  • Spanish language proficiency (oral and written).
  • Experience working with culturally diverse populations and knowledge of trauma-informed care and health equity principles.

Responsibilities

  • Conduct comprehensive physical, behavioral, safety, and psychosocial assessments during in-home or approved virtual visits.
  • Identify client and family strengths, needs, and social determinants of health to develop individualized nursing diagnoses and care plans with achievable goals, objectives, and activities.
  • Coordinate, monitor, and evaluate health services, treatment plans, and referrals to ensure continuity of care, optimal health outcomes, and the removal of care barriers.
  • Establish trusting, supportive relationships with families, acting as a dedicated advocate.
  • Educate and empower clients to problem-solve and make informed health decisions.
  • Deliver tailored education and counseling on health promotion, injury prevention, disease prevention, self-management strategies, and healthy family practices.
  • Create precise, timely documentation in electronic health records (EHR) in compliance with agency policy, grant requirements, HIPAA regulations, and Board of Nursing standards.
  • Develop effective working relationships with families, community agencies, and multidisciplinary healthcare professionals.
  • Participate in case conferences, quality assurance activities, and performance improvement initiatives.
  • Maintain flexible scheduling for occasional evening or weekend visits, participate in public health emergency response activities as directed, and perform other public health duties as assigned.
