The Public Health Educator & Navigator will promote, maintain, and improve individual and community health and well-being by designing and implementing public health education and outreach programs with a public health focus on primary prevention (including chronic disease prevention, tobacco education, etc.) and community behavioral health service navigation, promotion and support. This role will actively engage the community, lead public awareness efforts, stigma reduction and provide essential navigation and referral coordination to connect at-risk and underserved populations. The incumbent will collect and analyze data to identify community needs prior to planning, implementing, monitoring and evaluating programs to encourage health lifestyles, policies and environments. This is a grant-funded position, and employment is contingent upon the continued availability of grant funds. The primary work location is the Delta County office, with occasional off-site duties. Employees are expected to perform the majority of their duties from the designated office location.
