About The Position

ACTION-Housing, Pittsburgh's largest non-profit developer, is dedicated to improving lives through quality, affordable housing and fostering self-sufficiency. Since 1957, we have served diverse populations in Southwestern Pennsylvania, including seniors, veterans, individuals with disabilities, those on the autism spectrum, and youth aging out of foster care. The MyPlace PTI Program provides transitional housing for young adults aged 18-23 who are in or have aged out of foster care. The PTI Case Manager will offer intensive case management services to help participants achieve self-sufficiency and secure permanent housing.

Requirements

  • A degree in Social Services is required.
  • Experience in youth work is required.

Responsibilities

  • Develop and maintain positive relationships with program participants.
  • Teach and model independent living skills.
  • Provide case management services including school, employment, referrals, and contacts.
  • Maintain a physically and emotionally safe environment.
  • Meet with clients in their units, in the office, and in the community several times a month.
  • Attend and participate in monthly life skills classes with My Place staff and clients.
  • Assist in securing housing for program participants by visiting apartments throughout the Pittsburgh area and ensuring their cleanliness and safety.
  • Act as the Landlord by collecting rent payments from participants during the summer months and completing monthly unit inspections.
  • Develop, monitor, and revise, in conjunction with the client, the individual’s goal plan, housing plan, and monthly budget.
  • Assist with career development by identifying college majors/minors, plans for degree utilization, and providing connections to internships and professional development programs.
  • Develop a discharge plan with the client, including a two-month aftercare plan for program graduates.
  • Assist in securing permanent housing before program discharge.
  • Document casework activities and client interactions through written case notes.
  • Establish and maintain relationships with Independent Living Program (ILP) agencies in Allegheny County.
  • Establish and maintain relationships with other service providers as needed.
  • Prepare all reports as required by funding agencies.
  • Perform other responsibilities as assigned.
