About The Position

Change Grow Live is a charity committed to making a difference in people’s lives by creating safe, respectful, and empowering environments. A new opportunity has arisen for a Project Manager to lead the delivery and service development of CGL Nottinghamshire's RSDAT Assertive Outreach Team. This role is part of the Severe Multiple Disadvantages management team, focused on improving outcomes for individuals affected by substance use in Nottinghamshire. The position is full-time (37.5 hours per week) and is a fixed-term contract until March 31st, 2028.

Requirements

  • Experience engaging with individuals with severe multiple disadvantages.
  • Determined approach to finding solutions.
  • Comprehensive knowledge of one or more of the following: Homelessness, Mental health, Criminal Justice System, Drugs and/or alcohol use.
  • Considerable experience managing a team within the health & social care sector.
  • Proven track history of delivering quality services.
  • Demonstrable understanding of performance management frameworks, effective governance, and quality management.
  • Experience in the implementation and development of new services, evidencing a broad understanding of pathway integration.
  • Knowledge and understanding of possible recovery pathways across whole treatment and recovery systems (community, residential, prison).
  • Experience of staff supervision and appraisal.
  • Well-developed leadership skills.
  • Experience of data collection systems and information technology.
  • Comprehensive knowledge of severe multiple disadvantages, including barriers to support.
  • Possess, or be working towards a recognised qualification in health, social care, or management.
  • Must have the right to work in the UK.
  • This role is not eligible for visa sponsorship.

Responsibilities

  • Engage with people across Nottinghamshire who have a history of rough sleeping and substance use.
  • Lead the development of outreach provision, building a holistic service for individuals with severe multiple disadvantages.
  • Embed trauma-informed practice and ensure staff are well-trained and supported, including supervision and appraisal.
  • Map and coordinate outreach provision across the county, ensuring equitable resource allocation.
  • Participate in and lead multi-agency/disciplinary forums to identify gaps and improvements.
  • Achieve contractual output targets and produce timely reports to meet contractual, administrative, and financial requirements.
  • Provide line management, supervision, and leadership to achieve performance outcomes.
  • Identify areas for service development in conjunction with the Service Manager and Senior Management Team.
  • Oversee the monitoring and evaluation of the service and engage with service users, families, and carers on service development.
